Malaysian HO Application

The right procedure for HO application needs u to go putrajaya (SPA,MMC,MOH) atleast 2 days ( frstly,u wud need to reg at MMC days bfore n then come another day to SPA for intview n MOH )..but if ure lazy, far away, or u don’t have much time , this is the ideal one..u can just spend a day in putrajaya..

1st step
-fill SPA8i online (make sure u already hve full transcript)..in a week, SPA will send a letter of confrmation that u already been registerd..

-Download MMC form n fill (cont 4th step)

2nd step
If u r ready for appointment, then call the officer in charge 03-8885 6338/6372 (pn zamzarina/azura)..normally the interview will be held around 1-4 weeks after app hve been made...make sure u CALL . SPA will send an offer ltter via pos n emel as well..

3rd step
Interview at SPA. Come as early as 8am!! Ist come, 1st intrview, then ull hve more time to deal with MMC and MOH in the same day…Don’t worry about the interview,there will be 2 panel from spa n MOH respectively…bace dengue,DM, HT n ACS..cukuplah.....somehow u’ll pass..hehe

Suruhanjaya Perkhidmatan Awam Malaysia Aras 6-10, Blok C7, Kompleks C, Pusat Pentadbiran Kerajaan Persekutuan 62520 PUTRAJAYA MALAYSIA.

Make sure u bring:
i) 1 Passport picture
ii) Bring the ori & 1 certified copy of the following documents: – IC – Birth Cert – SPM Cert – Full Transcript (make sure translated to English) – Degree (Sked,MD)/ if u don’t hve dgree bring surat keterangan kelulusan
iii) Parents’ IC number

4th step
Hand in your form downloaded at 1st step at MMC
Malaysian Medical Council Bahagian Sumber Manusia Aras 2, Blok E1, Kompleks E, Pusat Pentadbiran Kerajaan Persekutuan, 62590 Putrajaya, Malaysia.

5th step
Go to Kompleks E. fill in your form(KKM)/ BMD (boring maklumat diri).. u would need to come in person to KKM to fill in this form.
Kementerian Kesihatan Malaysia Bahagian Sumber Manusia Aras 9, Blok E7, Kompleks E, Pusat Pentadbiran Kerajaan Persekutuan, 62590 Putrajaya, Malaysia.

i) 1 Passport picture
ii) Bring the original & 1 certified copy of the following documents: – IC – Birth Cert – SPM Cert – Full Transcript – Degree (Sked,MD)/ surat keterangan
iii) Parents’ IC number

6th step
Kursus induksi will be held around 1-2 mo after u get surat pendaftran sementara from MMC..u’ll know ur hospital placement there..

p/s bring original document and at least 5 certified copies of (IC,bith cer, spm cer, full transkrip,dgree/surat keterangan)
